Appointment Policy for Minors

Teenagers under 19 years of age are minors and should be accompanied by a parent or guardian if possible. Paper work and questionnaires brought to the office should be signed by a guardian. Patients 19 years and older

 
Because of federal privacy laws, we will now be requiring patients age 19 and older to sign a permission form which will allow us to speak to another person about their health care.  This form is posted on our web site under “Forms and Policies” and the name of the form is “Permission for verbal communications”.  THIS APPLIES ONLY TO PATIENTS WHO ARE 19 YEARS OF AGE AND OLDER.
